Your next nursing opportunity starts here! We are seeking a dedicated Registered Nurse with strong OASIS proficiency to fulfill a 13-week full-time contract in home health. Schedule:

• 13-week contract
• Full-time, Monday through Friday
• On-call duties may be required
• Daily visit expectations

Key Responsibilities:

• Deliver comprehensive in-home nursing care and skilled interventions
• Conduct detailed patient assessments and complete OASIS documentation accurately and promptly
• Collaborate with physicians and other healthcare team members to create and implement individualized care plans
• Provide wound care, oversee medication administration, offer disease monitoring, and educate patients
• Update clinical teams regarding patient status and changes in condition

Qualifications:

• Current and active RN license
• At least one year of recent home health nursing experience
• Demonstrated OASIS documentation expertise
• Proficiency in HomeCare HomeBase (strongly preferred)
• Reliable transportation and a valid drivers license
• Proven ability to manage a consistent, full-time visit schedule
