A highly skilled Registered Nurse with OASIS experience is needed for a 13-week full-time home health contract. This role calls for a confident clinician who excels in managing a set schedule, maintaining documentation accuracy, and delivering high-quality patient care in a home setting. Schedule:
- Monday through Friday, full-time hours
- On-call rotation may be expected
- Structured daily patient visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions to patients
- Complete patient assessments and OASIS documentation with accuracy and timeliness
- Develop and implement individualized care plans by collaborating with physicians and multidisciplinary teams
- Provide wound care, medication management, disease monitoring, and patient education
- Proactively communicate updates regarding patient conditions to the clinical care team
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proficiency with OASIS documentation is required
- Experience with HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to manage a structured schedule and meet daily visit requirements
