Advance your nursing career by providing essential care and support to patients in their homes. This full-time, 13-week contract offers the chance to deliver skilled clinical care in a field-based setting, ensuring patients receive the highest standard of treatment and education as they recover or manage chronic conditions.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, with daily travel to patient homes (mileage reimbursement provided)
• Caseload of 30 points per week, typically 35 patient visits per day

Qualifications:

• Active RN license in good standing for the state of practice
• Minimum of one year of recent home health nursing experience (field experience preferred)
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• OASIS assessment experience required
• Reliable transportation, valid drivers license, and auto insurance

Key Responsibilities:

• Conduct comprehensive OASIS assessments and establish individualized care plans
• Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and disease management
• Monitor patient progress, recognize changes in condition, and effectively communicate with interdisciplinary team members
• Educate patients and families on conditions, medications, and care strategies
• Maintain accurate, timely electronic documentation in HCHB
• Collaborate with therapists, social workers, and home health aides to coordinate holistic care
• Participate in case conferences, staff meetings, and ongoing education opportunities
• Comply with all documentation and care delivery standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
