A dedicated Registered Nurse with OASIS experience is needed for a 13-week full-time contract in home health. This is a chance to leverage your clinical expertise, ensure accurate documentation, and provide outstanding patient care in a supportive professional environment.
Schedule:
- 13-week assignment, potential for extension
- Monday through Friday, full-time hours
- Likely on-call responsibilities
- Daily structured visit expectations
Key Responsibilities:
- Deliver skilled, in-home nursing care and interventions
- Conduct comprehensive patient assessments and complete timely, accurate OASIS documentation
- Collaborate with physicians and multidisciplinary teams to develop and implement individualized care plans
- Manage wound care, medication administration, patient education, and ongoing monitoring of health conditions
- Clearly communicate updates or changes in patient status to the broader clinical team
Qualifications:
- Current, active RN license
- At least one year of recent home health RN experience
- Proven OASIS documentation expertise
- Proficiency with HomeCare HomeBase (strongly preferred)
- Capable of managing a full-time, structured schedule with multiple daily visits
- Reliable transportation and valid drivers license
