Schedule:

  • 40 hours per week
  • MondayFriday, 8:00 am 4:30 pm
  • Weekend call rotation: average one weekend/month, 12 overnight calls/week (4:30 pm to 8:00 am)
  • Rotating holiday coverage

Qualifications:

  • Active RN license required (AK license preferred; must apply within 24 hours of acceptance if not current)
  • BLS certification required
  • At least one year of recent experience with CMS/OASIS Home Health programs (two years preferred)
  • Reliable transportation is a must

Responsibilities:

  • Deliver skilled nursing care to patients in their homes, ensuring high standards consistent with CMS/Home Health regulations
  • Complete and document comprehensive assessments, OASIS evaluations, and care plans
  • Collaborate with interdisciplinary teams to optimize patient outcomes
  • Participate in on-call rotation, providing exceptional care after hours and during scheduled holidays
