Schedule:
- 40 hours per week
- MondayFriday, 8:00 am 4:30 pm
- Weekend call rotation: average one weekend/month, 12 overnight calls/week (4:30 pm to 8:00 am)
- Rotating holiday coverage
Qualifications:
- Active RN license required (AK license preferred; must apply within 24 hours of acceptance if not current)
- BLS certification required
- At least one year of recent experience with CMS/OASIS Home Health programs (two years preferred)
- Reliable transportation is a must
Responsibilities:
- Deliver skilled nursing care to patients in their homes, ensuring high standards consistent with CMS/Home Health regulations
- Complete and document comprehensive assessments, OASIS evaluations, and care plans
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Participate in on-call rotation, providing exceptional care after hours and during scheduled holidays
