Advance your nursing career while making a meaningful impact on patients lives through home health care. This full-time, 13-week contract role is ideal for a dedicated RN who excels at delivering personalized, field-based care within the comfort of patients homes.

Schedule & Work Environment:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based; daily travel required within assigned territory (mileage reimbursement provided)
• Caseload of 30 points weekly (typically 35 patient visits daily)

Primary Responsibilities:

• Conduct comprehensive assessments using OASIS and develop individualized care plans.
• Deliver a broad scope of skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management.
• Monitor and evaluate patient conditions; communicate promptly with the care team and physicians regarding any status changes.
• Provide education to patients and families regarding disease processes, medications, and home care best practices.
• Maintain accurate and timely documentation within Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, social workers, and home health aides for coordinated, high-quality patient care.
• Participate in staff meetings, case conferences, and ongoing education sessions.
• Ensure adherence to all regulatory and agency standards for documentation and care delivery.

Required Qualifications:

• Active RN license in good standing for the state of practice
• Minimum 1 year of recent home health experience (field experience highly desired)
• Proficient in Home Care Home Base (HCHB) electronic documentation and OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

$1,742 - $1,843/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
