We are seeking a skilled Registered Nurse with recent home health experience, including proficiency in OASIS charting, for a 13-week full-time contract position. Your ability to deliver compassionate care, manage a consistent workflow, and ensure accurate documentation is essential to thriving in this role.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be required
- Daily visit expectations apply
Responsibilities:
- Deliver in-home nursing care and skilled interventions tailored to patient needs
- Complete comprehensive OASIS assessments and documentation promptly and accurately
- Collaborate closely with physicians and interdisciplinary care teams to develop individualized care plans
- Provide wound care, medication management, disease monitoring, and patient/family education
- Maintain timely communication regarding changes in patient condition to the clinical team
Qualifications:
- Current, active RN license
- At least one year of recent experience in a home health setting as an RN
- Required OASIS documentation experience for Medicare-certified home health patients
- HomeCare HomeBase documentation system knowledge strongly preferred
- Ability to manage daily visits efficiently within a set schedule
- Reliable transportation and a valid driver's license
