Job Description

Deliver dedicated patient care as a Registered Nurse in a field-based home health setting. This full-time, 13-week contract involves providing skilled nursing to patients in their homes, focusing on assessment, care coordination, and ongoing education for patients and families.

Schedule and Details:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: Manage approximately 35 patient visits daily (30 points per week)
• Field-based: Daily travel within the assigned territory; mileage reimbursement available

Responsibilities include:

• Completing OASIS assessments and individualized care plans for each patient
• Delivering comprehensive nursing care, including wound care, medication administration, IV therapy, and chronic disease management
• Monitoring patient status, documenting progress, and communicating updates to physicians and the care team
• Educating patients and caregivers about treatment, medication regimens, and health management
• Maintaining thorough and timely documentation using Home Care Home Base (HCHB)
• Collaborating with therapy and social work professionals for holistic patient care
• Participating in staff meetings, case conferences, and professional development activities
• Ensuring compliance with all regulatory and agency documentation and care delivery standards

Preferred Qualifications:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent home health nursing experience, including field work
• Strong proficiency in HCHB and OASIS documentation
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
