Job Description

Providing skilled nursing care in patients homes, this Home Health Registered Nurse (RN) role requires strong clinical expertise and the ability to work independently. The successful candidate will perform thorough assessments, develop individualized care plans, and collaborate closely with interdisciplinary teams to deliver patient-focused care that promotes recovery and quality of life.

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop tailored care plans.
  • Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, identify clinical changes, and communicate effectively with physicians and care teams.
  • Educate patients and families on conditions, treatments, and preventive health.
  • Maintain timely, accurate documentation in Home Care Home Base (HCHB).
  • Collaborate with therapists, social workers, and aides to coordinate patient care.
  • Participate in case conferences, staff meetings, and continuing education.
  • Comply with regulatory standards for care and documentation.

Qualifications / Desired Experience:

  • Active RN license in good standing for the state of practice.
  • Minimum 1 year recent home health nursing experience; field experience preferred.
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessments required.
  • Strong clinical assessment and critical thinking skills.
  • Reliable transportation, valid drivers license, and current auto insurance.
  • Excellent communication, organizational skills, and ability to manage a field-based schedule independently.
  • BSN preferred.
  • Experience managing caseloads of 2532 points per week.
  • Knowledge of Medicare and home health compliance standards.

Schedule / Contract Details:

  • Full-time, 13-week contract.
  • Monday through Friday, 8:00 AM 5:00 PM.
  • Typically 3 to 5 patient visits daily, caseload of 2532 points per week.
  • Daily travel to patient homes within assigned territory (mileage reimbursement provided).
