Your next challenging and rewarding nursing opportunity awaits. We are seeking an experienced Registered Nurse with strong home health skills and OASIS expertise for a 13-week full-time contract. This is your chance to provide meaningful in-home care, supported by a team committed to professional excellence and patient well-being.
Schedule:
- Full-time, Monday through Friday
- On-call required as needed
- 13-week contract with the possibility of extension
Key Responsibilities:
- Deliver skilled nursing interventions and compassionate care in home settings
- Conduct thorough patient assessments and complete OASIS documentation promptly and accurately
- Collaborate with physicians and interdisciplinary team members to develop and adjust individualized care plans
- Perform wound care, medication administration, patient education, and monitor disease progression
- Maintain clear communication regarding changes in patient condition to ensure seamless continuity of care
Qualifications:
- Current, active Registered Nurse (RN) license
- At least 1 year of recent home health nursing experience
- Proficient with OASIS documentation
- Experience with HomeCare HomeBase (strongly preferred)
- Proven ability to manage daily patient visits and structured schedules
- Valid drivers license and reliable transportation
