Schedule:

• 13-week contract with potential for extension
• Full-time, Monday through Friday
• On-call responsibilities possible
• Daily visit expectations per agency guidelines

Responsibilities:

• Deliver in-home skilled nursing care and targeted interventions to patients
• Perform thorough admissions assessments and complete all OASIS documentation with precision
• Coordinate care with physicians and interdisciplinary teams, developing individualized care plans
• Provide wound care, oversee medication administration, educate patients and families, and monitor disease progression
• Immediately communicate any updates or changes in patient condition to the clinical team

Qualifications:

• Active RN license
• Minimum 1 year of recent experience as a home health RN
• Proficiency with OASIS documentation is required
• Familiarity with HomeCare HomeBase is strongly preferred
• Ability to maintain a consistent, structured schedule managing multiple daily patient visits
• Reliable transportation and a valid drivers license

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
