Schedule:
- Full-time, Monday through Friday
- On-call availability may be required
Qualifications:
- Active RN license
- Minimum 1 year of recent experience in home health as an RN
- Proficient in OASIS documentation
- Experience with HomeCare HomeBase strongly preferred
- Strong organizational and clinical skills
- Reliable transportation and valid drivers license
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions
- Complete patient assessments and submit timely, accurate OASIS documentation
- Manage a structured daily visit schedule
- Collaborate with physicians and interdisciplinary care teams
- Perform wound care, medication management, patient and family education, and monitor disease progress
- Communicate effectively regarding any changes in patient conditions

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
