We are seeking a dedicated Registered Nurse (RN) with proven Home Health expertise and OASIS assessment skills to provide compassionate care in a dynamic home health setting. This full-time, 13-week contract offers the chance to make a meaningful difference for patients while working alongside a collaborative team of professionals.

Schedule:

• 13-week contract with potential to extend
• Full-time hours, Monday through Friday
• On-call coverage may be required

Key Responsibilities:

• Deliver in-home nursing care and skilled clinical interventions
• Complete patient assessments and OASIS documentation accurately and within required timelines
• Coordinate with physicians and interdisciplinary teams to implement customized care plans
• Administer wound care, manage medications, educate patients, and oversee disease monitoring
• Update the clinical team on patient status changes or concerns as needed

Qualifications:

• Active and unencumbered RN license
• Minimum 1 year of recent experience as a home health RN
• Proficient in OASIS documentation (required)
• HomeCare HomeBase software experience strongly preferred
• Reliable transportation with a valid drivers license
• Commitment to a structured, full-time patient visit schedule
