We are hiring a dedicated Registered Nurse (RN) with OASIS expertise for a 13-week full-time contract serving patients in the Flagstaff, AZ area. This role is ideal for a motivated nurse who excels in independent practice, precise documentation, and compassionate patient care within the community.
Schedule:
- 13-week contract with possibility of extension
- Full-time, Monday through Friday
- On-call responsibilities are probable
- Daily visit expectations with mileage reimbursement provided
Your Role and Impact:
- Deliver skilled, in-home nursing care and patient-centered interventions
- Complete thorough patient assessments and precise, timely OASIS documentation
- Coordinate with physicians and interdisciplinary teams to tailor care plans
- Manage wound care, medication administration, patient education, and ongoing disease monitoring
- Provide clear, consistent communication regarding patient status and updates to the clinical team
Required Qualifications:
- Current, active RN license
- At least 1 year of recent home health experience as a Registered Nurse
- Proficient with OASIS documentation
- HomeCare HomeBase software proficiency strongly preferred
- Solid organizational skills to manage a full patient schedule
- Valid drivers license and reliable transportation
