Take on an impactful healthcare role where youll support patients in their homes with comprehensive nursing care. This full-time, 13-week contract position is designed for registered nurses who thrive in a field-based environment and excel at delivering quality care to a diverse patient population.

Schedule and Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: Daily travel to patient homes within a designated territory
• Caseload: 30 points per week (equivalent to approximately 35 patient visits daily)

Qualifications:

• Active RN license in good standing (state of practice)
• At least 1 year of recent, relevant Home Health nursing experience
• Proficient with Home Care Home Base (HCHB) electronic documentation
• Hands-on OASIS assessment experience
• Reliable transportation, a valid drivers license, and up-to-date auto insurance

Primary Responsibilities:

• Complete thorough OASIS assessments and design individualized patient care plans
• Deliver skilled care: wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, detect changes in condition, and communicate findings to care teams and physicians
• Educate patients and families on health conditions, treatment, medication, and self-care
• Maintain timely, accurate charts using Home Care Home Base (HCHB)
• Work closely with therapists, social workers, and aides to ensure coordinated patient-centered care
• Participate in case conferences, staff meetings, and required ongoing training
• Consistently adhere to regulatory and agency guidelines for clinical care and documentation

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
