Make a direct impact on patient outcomes by delivering compassionate care in a field-based home health setting. This full-time, 13-week contract opportunity is tailored for an experienced RN who thrives in an autonomous, patient-centered environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Caseload:

• 30 points per week (typically 35 patient visits per day)

Setting:

• Field-based: travel to patient homes within the assigned territory (mileage reimbursement included)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing interventions (wound care, IV therapy, medication management, and chronic disease support)
• Monitor patient status, recognize changes, and collaborate closely with physicians and the interdisciplinary team
• Educate patients and families on diagnoses, treatment regimens, and preventive strategies
• Ensure accurate, timely documentation within Home Care Home Base (HCHB)
• Collaborate with other disciplinesPT, OT, MSW, and home health aidesto achieve optimal patient outcomes
• Attend case conferences, staff meetings, and participate in ongoing educational opportunities
• Maintain compliance with all regulatory and agency documentation guidelines

Qualifications:

• Active RN license in good standing in the state of practice
• Minimum of 1 year recent home health experience (field-based preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Reliable transportation, a valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
