A rewarding position is available for a Registered Nurse (RN) with strong home health expertise and OASIS proficiency. This full-time 13-week assignment is designed for RNs who are confident in their clinical judgment, skilled in documentation, and thrive within a structured schedule while making a meaningful difference in patients lives.
Schedule:
- Full-time, Monday through Friday
- On-call rotation is likely
Key Responsibilities:
- Deliver in-home nursing care and perform skilled interventions
- Accurately complete patient assessments and OASIS documentation within established timelines
- Collaborate with physicians and fellow clinicians to create and implement tailored care plans
- Provide wound care, medication administration, disease monitoring, and comprehensive patient education
- Effectively communicate any changes in patient status to the care team
Essential Qualifications:
- Active RN license in good standing
- At least one year of recent home health RN experience
- Proficient with OASIS documentation (required)
- Experience with HomeCare HomeBase is strongly preferred
- Demonstrated ability to manage a set schedule and routine daily patient visits
- Valid drivers license and reliable vehicle
