Schedule:

• 13-week full-time contract with potential to extend
• Monday through Friday, with probable on-call assignments
• Structured daily visit expectations

Desired Qualifications:

• Active Registered Nurse (RN) license
• Minimum of 1 year recent RN experience in home health settings
• Proven OASIS documentation skills (required)
• HomeCare HomeBase proficiency (strongly preferred)
• Reliable transportation and valid drivers license
• Competence in managing a full caseload independently

Primary Responsibilities:

• Deliver comprehensive in-home nursing care and skilled interventions
• Accurately conduct and document patient assessments using OASIS
• Coordinate with physicians and interdisciplinary teams for customized care
• Administer wound care, medication, patient/family education, and chronic disease monitoring
• Effectively communicate patient status and needs to the clinical team
