Job Description

A dynamic opportunity is available for an experienced Registered Nurse specializing in home health care. This full-time, 13-week contract position offers a chance to provide high-quality, individualized nursing services to patients in a field-based setting, making a significant impact directly in their homes.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Expectation of 30 points per week (generally 35 home visits per day)
  • Daily travel within assigned territory (mileage reimbursement provided)

Key Qualifications:

  • Active and unrestricted RN license in state of practice
  • At least one year of recent home health experience, with field experience strongly preferred
  • Comfortable utilizing Home Care Home Base (HCHB) software and completing OASIS assessments
  • Reliable transportation, current drivers license, and valid auto insurance

Primary Responsibilities:

  • Complete thorough OASIS assessments to develop personalized care plans
  • Deliver direct nursing care, including wound care, medication management, IV therapy, and support for chronic conditions
  • Monitor patient progress, noting and communicating any changes to the interdisciplinary team
  • Provide education to both patients and families regarding diagnoses, medications, and preventive care
  • Maintain accurate, timely documentation using HCHB
  • Collaborate closely with physical and occupational therapists, social workers, and home health aides to ensure comprehensive care
  • Participate in case conferences, ongoing education, and staff meetings
  • Adhere to all regulatory and agency standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
