Step into a rewarding role providing compassionate care to patients in a home health setting for a 13-week, full-time contract. This position is ideal for an experienced RN who thrives in the field and enjoys building relationships with patients and families while delivering high-quality nursing services.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Details:

• Field-based position, visiting patients in their homes within an assigned territory
• Caseload of approximately 35 patient visits per day, totaling 30 points per week
• Utilizes Home Care Home Base (HCHB) electronic documentation system
• Daily travel with mileage reimbursement provided

Responsibilities:

• Conduct thorough OASIS assessments to develop personalized care plans
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
• Continuously assess patient progress, report changes, and coordinate with interdisciplinary team members
• Educate patients and families about treatment plans, medications, and health maintenance
• Maintain precise, timely documentation in HCHB
• Attend case conferences, staff meetings, and continuing education sessions as required
• Uphold all professional, regulatory, and agency standards

Qualifications:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent professional experience in Home Health; field-based work highly preferred
• Strong proficiency in Home Care Home Base (HCHB) and OASIS assessment tool
• Reliable transportation, valid drivers license, and proof of auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
