Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may apply
- Daily patient visit expectations
What Youll Do:
- Deliver in-home nursing care and skilled interventions tailored to each patient
- Perform thorough assessments and complete OASIS documentation with accuracy and timeliness
- Partner with physicians and other care professionals to coordinate effective, individualized care plans
- Administer wound care, conduct medication management, provide patient/family education, and monitor ongoing health status
- Relay significant changes and updates to the clinical team in a timely manner
Qualifications:
- Current active RN license
- At least one year of recent experience as a home health RN
- Proficiency in OASIS documentation is essential
- Familiarity with HomeCare HomeBase (preferred)
- Capable of managing a full-time, structured caseload with daily patient visits
- Must have reliable transportation and a valid drivers license
