Take on a vital role providing quality nursing care to patients in their homes as part of a dynamic home health team. This full-time, 13-week contract offers a consistent schedule and the chance to truly impact patient lives across the community.

Schedule & Work Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, visiting patient homes within an assigned territory
• Expect 30 points per week (around 35 patient visits daily)
• Daily travel required; mileage reimbursement provided

Qualifications:

• Active RN license in good standing for the state of practice
• At least 1 year of recent home health nursing experience (field experience preferred)
• Proficient in Home Care Home Base (HCHB)
• Experience using OASIS assessment tool
• Valid drivers license, reliable transportation, and current auto insurance

Key Responsibilities:

• Complete comprehensive OASIS assessments and design individualized care plans
• Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, report condition changes, and communicate with physicians and the care team
• Educate patients and families about diagnoses, medications, and treatment plans
• Maintain accurate, timely documentation using HCHB
• Collaborate with the interdisciplinary team to coordinate care
• Participate in case conferences, staff meetings, and ongoing education
• Ensure compliance with regulatory and agency standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
