Your next nursing opportunity starts here. Were seeking a compassionate and detail-oriented Registered Nurse with strong home health and OASIS documentation skills for a full-time 13-week assignment. This contract offers a dynamic environment where your expertise will directly support patients in achieving greater health and independence at home.
Key Details:
- Schedule: Full-time, Monday through Friday; on-call likely as part of the assignment
- 13-week contract, with potential for extension
- Daily structured patient visit expectations
Primary Responsibilities:
- Deliver comprehensive in-home nursing care and skilled clinical interventions
- Conduct assessments and complete OASIS documentation accurately and on schedule
- Partner with physicians and interdisciplinary teams to create and follow individualized care plans
- Provide wound care, manage medications, educate patients and families, and monitor chronic conditions
- Update and communicate patient status or changes to the clinical team as needed
Qualifications:
- Active Registered Nurse license (RN)
- At least 1 year of recent home health RN experience
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase is strongly preferred
- Valid drivers license and access to reliable transportation
- Ability to maintain a consistent, full-time schedule with daily patient visits
