Key Responsibilities:
- Deliver skilled in-home nursing care and clinical interventions
- Perform comprehensive patient assessments with accurate, timely OASIS documentation
- Coordinate with physicians and interdisciplinary teams to develop tailored care plans
- Provide wound care, medication management, patient and family education, and disease monitoring
- Communicate patient status updates effectively to the clinical team
Qualifications:
- Active Registered Nurse license
- At least 1 year of recent home health RN experience
- Demonstrated experience with OASIS documentation
- Familiarity with HomeCare HomeBase strongly preferred
- Reliable vehicle and valid drivers license
- Capable of maintaining a full-time schedule and consistent daily patient visits

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
