Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily visit expectations set for case management consistency
Key Responsibilities:
- Deliver skilled in-home nursing care with patient-centered focus
- Complete patient assessments and OASIS documentation thoroughly and efficiently
- Collaborate closely with physicians and interdisciplinary teams to develop personalized care plans
- Provide wound care, manage medications, offer patient and family education, and monitor chronic disease management
- Communicate changes in patient condition promptly to the clinical team
- Maintain accurate clinical records and ensure compliance with all regulations
Qualifications:
- Active RN license
- At least one year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation (required)
- Strongly prefer experience with HomeCare HomeBase software
- Able to manage a structured schedule and meet daily visit requirements
- Reliable transportation and valid drivers license
