A skilled RN is needed for a field-based home health opportunity on a 13-week full-time contract. Deliver exceptional, patient-centered care to individuals in their homes, utilizing your clinical expertise and advanced assessment skills in a rewarding, autonomous environment.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

Key Requirements:

• Current, active RN license in good standing for this state
• At least one year of recent home health experience, preferably with field visits
• Strong working knowledge of Home Care Home Base (HCHB) electronic documentation system
• OASIS assessment experience is a must
• Reliable transportation, valid drivers license, and current auto insurance

Role Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans for a caseload averaging 35 patient visits per day (30 points per week)
• Provide a full range of skilled nursing interventions such as wound care, medication administration, IV therapy, and management of chronic diseases
• Continuously monitor patient progress, recognize changes in condition, and communicate promptly with the care team and physicians
• Educate patients and caregivers on managing their condition, understanding medications and treatments, and maintaining optimal health at home
• Document care accurately and promptly within Home Care Home Base (HCHB)
• Collaborate with interdisciplinary providers, including therapists and home health aides, to ensure holistic, coordinated patient care
• Attend case conferences, team meetings, and participate in ongoing professional development

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
