This travel assignment will allow you to work as an integral care coordinator, providing and directing patient care according to physicians orders and established agency policies. Schedule/Hours
- Home visits as required; periodic office meetings
Required Qualifications & Experience
- Current, unrestricted Registered Nurse license valid in Maryland or qualifying state (compact licensure welcomed)
- Two to three years of RN experience, preferably in home health or community nursing
- Graduation from an accredited school of professional nursing (BSN strongly preferred)
- Valid drivers license, reliable transportation, and required liability insurance
- Current CPR certification
- Proficient with electronic documentation, emails, and basic computer functions
- Ability to assess and respond to age-specific patient needs
Key Responsibilities
- Perform initial assessments, develop and update comprehensive care plans
- Deliver and coordinate skilled nursing services in patients homes
- Supervise and evaluate home health aides and LPNs/LVNs per regulatory requirements
- Communicate patient status changes promptly to physicians and interdisciplinary teams
- Maintain detailed clinical records and progress notes
- Participate in ongoing staff development and in-service education
