This travel assignment will allow you to work as an integral care coordinator, providing and directing patient care according to physicians orders and established agency policies. Schedule/Hours

  • Home visits as required; periodic office meetings

Required Qualifications & Experience

  • Current, unrestricted Registered Nurse license valid in Maryland or qualifying state (compact licensure welcomed)
  • Two to three years of RN experience, preferably in home health or community nursing
  • Graduation from an accredited school of professional nursing (BSN strongly preferred)
  • Valid drivers license, reliable transportation, and required liability insurance
  • Current CPR certification
  • Proficient with electronic documentation, emails, and basic computer functions
  • Ability to assess and respond to age-specific patient needs

Key Responsibilities

  • Perform initial assessments, develop and update comprehensive care plans
  • Deliver and coordinate skilled nursing services in patients homes
  • Supervise and evaluate home health aides and LPNs/LVNs per regulatory requirements
  • Communicate patient status changes promptly to physicians and interdisciplinary teams
  • Maintain detailed clinical records and progress notes
  • Participate in ongoing staff development and in-service education
