Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Essential Qualifications:

• Active RN license in good standing for the state of practice
• Minimum one year of recent Home Health nursing experience (field experience highly preferred)
• Proficient with Home Care Home Base (HCHB) electronic documentation system
• Demonstrated experience completing OASIS assessments
• Reliable transportation, current drivers license, and auto insurance

Key Responsibilities:

• Complete comprehensive patient assessments using OASIS and develop tailored care plans
• Deliver skilled nursing care: wound care, medication management, IV therapy, chronic disease management, and more
• Monitor patient progress and communicate updates to the interdisciplinary care team and referring physicians
• Educate patients and families on disease processes, medications, and care strategies
• Document all care delivery promptly and accurately in HCHB
• Collaborate closely with physical therapists, occupational therapists, medical social workers, and home health aides for holistic care
• Join regular staff meetings, case conferences, and participate in continuing education opportunities
• Maintain compliance with all regulatory, documentation, and care delivery standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
