We are seeking a dedicated Registered Nurse with strong OASIS experience for a 13-week full-time contract, providing vital in-home care and helping patients achieve the best possible outcomes. Schedule:
- 13-week contract, potential for extension
- Full-time, Monday through Friday
- On-call availability probable
- Daily visit structure to manage your workflow
What Youll Do:
- Deliver compassionate nursing care and skilled interventions in patients homes
- Perform comprehensive patient assessments, completing timely and accurate OASIS documentation
- Collaborate closely with physicians and healthcare teams to develop and implement individual care plans
- Provide wound care, medication management, patient teaching, and monitoring of various conditions
- Keep the clinical team informed about updates or changes in patient status
What Were Looking For:
- Valid RN license in good standing
- At least one year of recent home health RN experience
- Required proficiency with OASIS
- Strongly prefer experience with HomeCare HomeBase
- Demonstrated ability to maintain a structured, full-time visit schedule
- Dependable transportation and a current drivers license
