Job Description

A skilled RN is needed for a full-time, 13-week contract providing home health care services. This dynamic field-based role serves patients in their homes, requiring daily travel within an assigned territory. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with a caseload expectation of 30 points per weektypically averaging 35 patient visits daily.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing care, such as wound care, medication administration, IV therapy, and chronic disease management
• Evaluate patient progress and communicate any status changes with the care team and physicians
• Educate patients and families on diagnosis, treatment plans, medications, and preventive strategies
• Document all visits thoroughly and promptly using Home Care Home Base (HCHB)
• Collaborate closely with a multidisciplinary team, including therapists, medical social workers, and home health aides
• Participate in case conferences, staff meetings, and ongoing education activities
• Comply with all regulatory and agency standards regarding documentation and care delivery

Qualifications:

• Active, unencumbered RN license applicable to the state of practice
• Minimum of one year recent experience in home health (field experience highly preferred)
• Strong proficiency with Home Care Home Base (HCHB)
• OASIS assessment experience required
• Valid drivers license, reliable vehicle, and proof of auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
