Your next nursing opportunity starts here. We're seeking a skilled Registered Nurse with proven Home Health and OASIS experience for a 13-week full-time contract. This role is ideal for clinicians who excel in patient care, meticulous documentation, and efficient schedule management within the home health setting.

Schedule:

• 13-week contract, potential for extension
• Full-time hours, Monday through Friday
• On-call duties likely as part of the rotation
• Daily structured patient visit assignments

Responsibilities:

• Deliver in-home nursing services and skilled care interventions to patients
• Conduct thorough patient assessments, completing all OASIS documentation accurately and promptly
• Work collaboratively with physicians and interdisciplinary care teams to develop and execute individualized care plans
• Perform wound care, medication administration, patient education, and disease monitoring
• Communicate important updates and changes in patient condition to the clinical team

Qualifications:

• Current and active RN license
• Minimum of 1 year of recent experience as a Home Health RN
• Proficient in OASIS documentation (required)
• Familiarity with HomeCare HomeBase (strongly preferred)
• Reliable transportation and a valid drivers license
• Ability to manage a consistent full-time patient load and schedule
