Job Description
Deliver exceptional patient care while enjoying the autonomy and flexibility of home health nursing. A dedicated Registered Nurse (RN) is needed for a contract role, providing skilled clinical support to individuals in their homes. Your expertise will ensure continuity of care, improved patient outcomes, and a supportive experience for families.
Schedule/Hours:
- Contract-based position
Key Responsibilities:
- Deliver comprehensive nursing care in home settings, including assessments, vital signs, and wound care
- Conduct initial and ongoing evaluations to develop and update individualized care plans
- Manage medications, perform IV therapy, and provide patient and family education on disease management and preventative care
- Communicate and coordinate closely with physicians, therapists, and interdisciplinary team members
- Ensure timely, accurate documentation of all patient interactions using electronic medical records (EMR)
- Monitor patient conditions, reporting significant changes promptly
- Adhere to all regulatory standards for home health care
Qualifications:
- Active RN license
- At least 3 months of recent home health experience utilizing OASIS
- Familiarity with HomeCare HomeBase (HCHB) is highly valued
- Strong clinical assessment skills and experience with wound care, IV therapy, and medication administration
- Proficiency with EMR platforms
- Excellent communication and time management abilities
- Ability to work autonomously and visit patients reliably (valid drivers license and transportation required)
