Advance your nursing career by making a direct impact on patient outcomes in a home health setting. This full-time, 13-week contract role focuses on providing high-quality care and support to patients in their residences.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

In this field-based position, you will manage a caseload averaging 35 patient visits daily and utilize your expertise to promote patient independence and health improvement. Mileage reimbursement is provided for daily travel within your assigned territory.

Qualifications:

• Current, active RN License (in good standing) in the state of practice
• At least one year of recent Home Health experience, preferably in the field
• Demonstrated proficiency with Home Care Home Base (HCHB)
• Previous experience with OASIS assessment and documentation
• Reliable transportation, valid drivers license, and auto insurance

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient status, communicate any changes to the care team and physicians promptly
• Educate patients and families regarding diagnoses, medications, treatment plans, and preventive health measures
• Maintain thorough, timely, and accurate documentation within HCHB software
• Work closely with interdisciplinary team members to ensure holistic patient care
• Attend case conferences, staff meetings, and participate in ongoing education

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
