A rewarding opportunity awaits a Registered Nurse with home health experience to provide compassionate, skilled nursing care in a field-based setting. This full-time, 13-week contract role involves visiting patients in their homes to deliver comprehensive nursing services and coordinate ongoing care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans for each patient.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, recognize changes in condition, and communicate promptly with physicians and team members.
• Educate patients and families about health conditions, care plans, and preventive strategies.
• Maintain accurate, timely clinical documentation in Home Care Home Base (HCHB).
• Work collaboratively with physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and continuing education activities.
• Ensure compliance with all clinical and regulatory standards.

Qualifications:

• Active RN license in good standing in the state of practice.
• At least one year of recent home health experience, preferably in a field-based role.
• Proficient in Home Care Home Base (HCHB) documentation system.
• Experience performing OASIS assessments.
• Reliable transportation, a valid drivers license, and current auto insurance.

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
