Take your nursing expertise out into the community with a dedicated home health assignment focused on supporting patients directly in their homes. This full-time, 13-week contract offers the opportunity to make a tangible difference, working Monday through Friday from 8:00 AM to 5:00 PM. Each week, youll balance a caseload of approximately 30 points, typically covering 35 patient visits daily within your assigned local territory.
Key Responsibilities:
- Conduct thorough OASIS assessments and develop personalized care plans
- Deliver skilled nursing care including wound care, medication administration, and IV therapy
- Monitor patient conditions, document progress, and coordinate promptly with the interdisciplinary care team
- Educate patients and families regarding condition management, medications, and healthy practices
- Enter timely, accurate patient records using Home Care Home Base (HCHB)
- Collaborate effectively with therapists, social workers, and aides to provide holistic patient care
- Attend regular case conferences, staff meetings, and participate in ongoing training
- Maintain compliance with clinical and documentation standards
Requirements:
- Active, unencumbered RN license in the state
- Minimum one year of recent Home Health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS
- Valid drivers license, reliable vehicle, and current auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
