We are seeking an experienced Registered Nurse (RN) who excels in clinical practice and documentation, specifically OASIS, for a 13-week full-time contract. Schedule:
- 13-week full-time contract, with the potential for extension
- Monday through Friday schedule (on-call likely as needed)
- Structured daily visit assignments
Key Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience as a home health RN
- Strong proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase is highly preferred
- Reliable transportation and a valid drivers license
- Able to consistently manage full-time daily patient visits
Duties and Responsibilities:
- Provide skilled in-home nursing interventions and patient care
- Complete comprehensive patient assessments and timely OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams
- Deliver wound care, medication management, patient/caregiver education, and monitor disease progress
- Accurately report condition changes and updates to the clinical team
