Schedule:

- Full-time, Monday through Friday

What You'll Do:
- Deliver skilled in-home nursing care and interventions
- Complete patient assessments and OASIS documentation accurately and promptly
- Work closely with physicians and interdisciplinary teams to develop individualized care plans
- Perform wound care, medication management, patient education, and chronic disease monitoring
- Communicate effectively with the clinical team, providing updates and recommendations
Ideal Candidate Profile:
- Current, active RN license
- At least one year of recent home health RN experience
- Proven OASIS proficiency (required)
- Experience with HomeCare HomeBase strongly preferred
- Strong organizational skills and reliability with schedules and daily visits
- Valid drivers license and dependable transportation
