Schedule:

• Full-time, Monday through Friday
• On-call availability may be required

Key Responsibilities:

• Deliver high-quality in-home skilled nursing services
• Complete patient assessments and OASIS documentation accurately and punctually
• Collaborate with healthcare providers to implement individualized care plans
• Administer wound care, medication management, patient teaching, and disease monitoring
• Provide consistent updates regarding patient status to the clinical team

Qualifications:

• Active RN license
• At least one year of recent home health RN experience
• Proven OASIS documentation proficiency
• Familiarity with HomeCare HomeBase (preferred)
• Ability to manage a structured, full-time schedule and daily patient visits
• Reliable transportation and a valid drivers license
