A full-time, 13-week field-based assignment is available for a skilled registered nurse experienced in home health care. This role is crucial in supporting patients within their homes, providing high-quality, personalized care, and ensuring optimal recovery and health outcomes.

Schedule & Hours:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN license in good standing (state of practice)
• At least 1 year of recent home health nursing experience, preferably in a field setting
• Demonstrated proficiency with Home Care Home Base (HCHB) electronic documentation
• OASIS assessment experience
• Reliable transportation, valid drivers license, and current auto insurance

Primary Responsibilities:

• Perform comprehensive OASIS assessments and formulate individualized patient care plans
• Deliver skilled nursing interventions (e.g., wound care, IV therapy, medication administration, chronic disease management)
• Monitor patient progress, promptly identify any changes, and communicate with the interdisciplinary care team and physicians
• Educate patients and families on disease management, medications, and preventive care techniques
• Maintain accurate, timely documentation using Home Care Home Base (HCHB)
• Collaborate with PT, OT, medical social workers, and home health aides to coordinate holistic care
• Participate in case conferences, staff meetings, and ongoing professional development
• Uphold all regulatory requirements and organizational standards for documentation and care delivery
• Daily travel within assigned territory (mileage reimbursement offered)

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
