Deliver compassionate patient care in a home health setting, providing essential nursing support to individuals recovering or managing chronic conditions in their own environments. As a Registered Nurse specializing in Home Health, your expertise will play a central role in helping patients achieve the best possible quality of life outside the hospital.
Schedule/Hours
- Contract position
- Flexible scheduling (hours will be discussed during interview)
Key Qualifications:
- Current, active RN license
- Minimum of one year of recent experience in home health nursing
- Ability to work independently and manage a patient caseload
- Strong clinical assessment and documentation skills
- Proficient with electronic health records
- Valid driver's license and reliable transportation
- Exceptional interpersonal, organizational, and communication skills
- CPR certification (AHA or equivalent preferred)
Role Responsibilities:
- Provide skilled nursing assessments and direct patient care in private homes
- Develop and update individualized care plans based on patient status and needs
- Administer medications, treatments, and procedures per physician orders
- Educate patients and family members on ongoing care, disease processes, and wellness strategies
- Monitor patients progress toward goals, reporting significant changes to the interdisciplinary team
- Maintain accurate, timely documentation of all care provided
- Collaborate with physicians, therapists, and social workers as needed to ensure coordinated care
- Participate in on-call rotation or weekends as required
