As a Registered Nurse specializing in Home Health, your expertise will play a central role in helping patients achieve the best possible quality of life outside the hospital.

Schedule/Hours

• Contract position
• Flexible scheduling (hours will be discussed during interview)

Key Qualifications:

• Current, active RN license
• Minimum of one year of recent experience in home health nursing
• Ability to work independently and manage a patient caseload
• Strong clinical assessment and documentation skills
• Proficient with electronic health records
• Valid driver's license and reliable transportation
• Exceptional interpersonal, organizational, and communication skills
• CPR certification (AHA or equivalent preferred)

Role Responsibilities:

• Provide skilled nursing assessments and direct patient care in private homes
• Develop and update individualized care plans based on patient status and needs
• Administer medications, treatments, and procedures per physician orders
• Educate patients and family members on ongoing care, disease processes, and wellness strategies
• Monitor patients progress toward goals, reporting significant changes to the interdisciplinary team
• Maintain accurate, timely documentation of all care provided
• Collaborate with physicians, therapists, and social workers as needed to ensure coordinated care
• Participate in on-call rotation or weekends as required
