A Registered Nurse with home health expertise and OASIS experience is needed for a 13-week full-time contract in the Derry, NH area. This role demands strong clinical skills, precise documentation, and excellent schedule management to deliver high-quality in-home care.

Responsibilities:
- Provide skilled nursing care and interventions in patients homes
- Conduct thorough patient assessments and complete accurate OASIS documentation on schedule
- Collaborate effectively with physicians and care teams to develop and implement individualized care plans
- Administer wound care, manage medications, educate patients, and monitor chronic conditions
- Communicate promptly with the clinical team regarding changes in patient status
Qualifications:
- Active Registered Nurse (RN) license
- Minimum of one year recent experience in home health nursing
- Demonstrated proficiency with OASIS documentation (required)
- Familiarity with HomeCare HomeBase software is strongly preferred
- Ability to manage a structured full-time schedule with daily patient visits
- Reliable transportation and a valid drivers license
Contract Details:
- 13-week contract with potential for extension
- Full-time, Monday through Friday schedule with probable on-call responsibilities
