Schedule/Hours
- Flexible weekday scheduling
- No mandatory weekend rotation (opportunities to work weekends available)
- 13-week schedule cycles
- Holiday coverage required for approximately 75% of major dates
Qualifications
- Active RN license
- Minimum of 1 year home health nursing experience
- Homecare Homebase (HCHB) EMR proficiency (required)
- Valid drivers license and current auto insurance
- BLS certification
- Strong skills in IV management, pleurx and wound vac care, OASIS, patient education, and episode management (managing up to 25-30 patients)
Responsibilities
- Complete a minimum of 6 patient visits daily, managing scheduling day prior
- Assess, discharge, recertify, and reassess patients in home settings
- Deliver specialized wound care and chronic disease management
- Coordinate case management and collaborate with an interdisciplinary team
- Utilize EPIC Rover (view only), Focura, and Samsung Note 9 for documentation and communications
