Schedule/Hours
- Flexible scheduling with a 13-week cycle
- No mandatory weekend rotation, unless desired
- Expected to work 75% of major holidays
- Manager-approved requested time off
Qualifications
- Current RN license, BLS certification
- Valid drivers license and auto insurance
- Minimum of 1 year of recent Home Health RN experience
- Proficiency with Homecare Homebase (HCHB) electronic recordsno exceptions
- Experience with IVs, pleurx, wound vacs, wound care, assessments, and OASIS documentation
- Strong case management skills (manage up to 2530 patients/episodes)
Responsibilities
- Complete six patient visits per day (routine, discharge, ROC, recert, reassessment)
- Provide teaching, wound care, chronic disease management, and home-based assessments
- Coordinate patient care and schedule visits one day ahead
- Travel an average of 6080 miles daily (mileage reimbursed weekly)
- Document patient care using provided Samsung Note 9 phones and HCHB
