A fulfilling 13-week full-time contract is available for a skilled Registered Nurse to deliver excellence in home health care. If youre experienced with OASIS documentation and thrive in an autonomous, patient-centered environment, this is your chance to truly make a difference within the community.
Schedule:
- 13-week contract, with the potential to extend
- Full-time: Monday through Friday
- On-call responsibilities may be required
Primary Responsibilities:
- Deliver in-home RN-level care and skilled interventions to patients
- Complete timely, accurate patient assessments with thorough OASIS documentation
- Collaborate with referring physicians, interdisciplinary team members, and families to develop and implement individualized care plans
- Provide wound care, medication management, patient/caregiver education, and monitor chronic disease progression
- Keep the clinical team informed with timely updates on patient status or changes
- Travel between homes, providing compassionate care and support in each setting
Preferred Qualifications:
- Active, unencumbered RN license
- At least 1 year of recent experience as a home health RN
- Proficient in OASIS documentation (required)
- Proficiency with HomeCare HomeBase (highly preferred)
- Demonstrated ability to manage a busy, structured daily visit schedule
- Reliable transportation and a valid drivers license
