Your next career step in home health nursing starts today. We're seeking a dedicated Registered Nurse (RN) with strong OASIS skills for a 13-week full-time contract assignment. This role is ideal for motivated professionals who excel at autonomy, value accurate documentation, and thrive when delivering high-quality care to patients in their homes.
Schedule:
- Full-time assignment, Monday through Friday
- Potential on-call responsibilities
Key Responsibilities:
- Deliver in-home patient care and skilled nursing interventions in accordance with physician orders
- Perform patient assessments and ensure timely, precise OASIS documentation
- Collaborate with physicians and interdisciplinary care teams to individualize patient plans of care
- Provide wound care, oversee medication management, educate patients, and monitor chronic conditions
- Promptly relay changes in condition or care needs to the clinical team
Qualifications:
- Active, unencumbered RN license
- Minimum of one year recent experience in home health nursing
- Proven OASIS assessment and documentation experience required
- Proficiency with HomeCare HomeBase (strongly preferred)
- Capable of managing a full, structured schedule with daily visits
- Reliable transportation and valid drivers license
