An experienced Registered Nurse (RN) with a strong background in home health is sought for a contract position. This role requires a dedicated professional passionate about delivering high-quality patient care in a home setting. OASIS experience is essential for success in this position.
Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes.
- Conduct initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and their families on disease management, treatment plans, and preventive care.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive care.
- Document patient care activities accurately and promptly in electronic medical records (EMR).
- Monitor and report changes in patient condition to the supervising physician.
- Ensure compliance with all applicable state and federal regulations related to home health care.
Qualifications:
- Active RN license.
- Minimum of three months of home health nursing experience using OASIS.
- Experience with HCHB is highly desirable.
- Strong clinical skills, including IV therapy, wound care, and medication management.
- Proficient in electronic medical records (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid drivers license and reliable transportation.
