A rewarding opportunity is available for an experienced Registered Nurse (RN) specializing in home health care to provide compassionate, patient-centered services on a contract basis. This role focuses on delivering high-quality nursing care directly in patients' homes, ensuring comfort and personalized attention.
Key Responsibilities:
- Conduct thorough initial and ongoing patient assessments, including vital signs, medication administration, and wound care.
- Develop and execute customized care plans tailored to each patients unique needs.
- Educate patients and their families about disease management, treatment plans, and preventive healthcare.
- Collaborate with physicians, therapists, and other healthcare professionals to maintain a comprehensive and cohesive care strategy.
- Accurately document all patient care activities in electronic medical records (EMR).
- Monitor patient conditions closely and communicate any significant changes to supervising physicians.
- Ensure strict compliance with all state and federal regulations governing home health services.
Qualifications:
- Active Registered Nurse (RN) license.
- At least 3 months of home health nursing experience with demonstrated proficiency in OASIS documentation.
- Experience with HomeCare HomeBase (HCHB) software is highly desirable.
- Strong clinical skills including IV therapy, wound care, and medication management.
- Proficient with electronic medical record (EMR) systems.
- Excellent communication and interpersonal skills.
- Ability to work independently while managing time effectively.
- Valid driver's license and reliable transportation to travel to patient homes.
