As a registered nurse within the home health field, youll play a pivotal role in supporting patients recovery and well-being outside of a clinical facility, delivering individualized, high-quality nursing care directly in their homes.
Qualifications and Experience:
- Current and valid RN license
- At least one year of experience in home health nursing preferred
- Strong assessment and critical thinking abilities
- Proficiency in electronic medical records and documentation
- Excellent communication and interpersonal skills
- Ability to operate independently while collaborating with an interdisciplinary team
- Reliable transportation and valid drivers license
Key Responsibilities:
- Conduct initial and ongoing patient assessments
- Develop and implement customized care plans tailored to each patients needs
- Administer medications and perform wound care, injections, and other nursing interventions
- Educate patients and family members on disease processes, medication management, and preventative care
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate holistic care
- Monitor and document patient progress, updating care plans as needed
- Ensure compliance with all regulatory requirements and standards of practice
