Advance your nursing career with a field-based role supporting patients in their homes. This full-time, 13-week contract position is designed for a Registered Nurse confident in home health care, providing specialized nursing interventions and acting as an integral part of an interdisciplinary team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Qualifications:

• Active RN License in the state of practice, maintained in good standing
• Minimum of 1 year of recent experience in Home Health (field experience strongly preferred)
• Proficient with the Home Care Home Base (HCHB) electronic documentation system
• Demonstrated experience completing OASIS assessments
• Reliable vehicle, current drivers license, and valid auto insurance

Essential Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans for patients
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and management of chronic conditions
• Monitor patient status, communicate changes to physicians and care team, and adjust care plans as needed
• Educate patients and their families about disease processes, medication regimens, and preventive health measures
• Document all patient care activities accurately and promptly in Home Care Home Base (HCHB)
• Work closely with physical therapists, occupational therapists, medical social workers, and home health aides to ensure high-quality, coordinated patient care
• Participate in staff meetings, case conferences, and ongoing education
• Uphold regulatory and agency standards for both patient care and recordkeeping
• Daily travel to patient homes within assigned territory (mileage reimbursement included)

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
