Take your nursing expertise into the field by delivering high-quality patient care in home settings. This full-time, 13-week contract role focuses on providing compassionate service and skilled nursing to patients within their own homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Responsibilities:

• Perform comprehensive OASIS assessments and develop tailored care plans for each patient.
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management during 35 daily home visits.
• Closely monitor patient progress, report changes, and coordinate updates with physicians and the broader healthcare team.
• Educate patients and their families about disease processes, medications, and effective self-care strategies.
• Maintain accurate and timely electronic documentation using Home Care Home Base (HCHB).
• Collaborate with other care providersincluding physical therapists, occupational therapists, social workers, and home health aidesto ensure comprehensive, coordinated care.
• Attend case conferences, staff meetings, and participate in ongoing training as needed.
• Follow all agency and regulatory standards for documentation and patient care.

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent field-based Home Health experience
• Strong proficiency in Home Care Home Base (HCHB) and experience with OASIS assessments
• Valid drivers license, reliable transportation, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
