Experience an impactful nursing opportunity delivering skilled care directly in patients homes. This full-time, 13-week contract offers a MondayFriday, 8:00 AM 5:00 PM schedule, providing a predictable work week. As a valued Registered Nurse in home health, youll support patient independence and recovery through field-based visits and compassionate clinical expertise.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM

Desired Qualifications:

• Current, active RN license (state of practice)
• At least 1 year of recent home health RN experience, with field experience highly preferred
• Expertise with Home Care Home Base (HCHB) for documentation
• Prior OASIS assessment and care planning experience
• Reliable transportation, valid drivers license, and auto insurance

Responsibilities:

• Complete OASIS assessments in patient homes and develop individualized care plans
• Deliver skilled nursing including wound care, IV therapy, medication administration, and chronic disease education
• Conduct daily patient visits (35 per day) within a designated territory, with total weekly caseload averaging 30 points
• Provide individualized patient and family education to support recovery and long-term wellness
• Maintain timely and accurate documentation using Home Care Home Base (HCHB)
• Collaborate with an interdisciplinary care team to coordinate patient services
• Participate in case conferences, team meetings, and ongoing learning as needed
• Travel daily within the assigned coverage areamileage reimbursement is provided

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
