Job Description

Embark on a rewarding nursing assignment delivering quality care to patients in their homes. This full-time, 13-week contract position offers you the opportunity to make a difference in a field-based setting as a Registered Nurse providing skilled care, education, and support to patients and families.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based with daily travel to patient homes within an assigned territory

Qualifications:

• Active RN license (in good standing)
• At least 1 year of recent home health nursing experience (field experience preferred)
• Experience with OASIS assessment and documentation
• Proficiency with Home Care Home Base (HCHB) electronic documentation system
• Reliable transportation, valid driver's license, and current auto insurance

Key Responsibilities:

• Conduct comprehensive OASIS assessments, developing individualized care plans for patients
• Deliver skilled nursing care, including wound management, medication administration, IV therapy, and chronic disease monitoring
• Monitor patient progress, recognize changes in condition, and promptly communicate updates to the care team and physicians
• Provide education to patients and families regarding diagnoses, treatment plans, medications, and preventative strategies
• Maintain up-to-date, accurate documentation in HCHB
• Collaborate with interdisciplinary team members to ensure cohesive, high-quality care
• Take part in case conferences, staff meetings, and ongoing professional development
• Meet regulatory and organizational standards for care delivery and record-keeping

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
