Your next opportunity to excel in home health nursing starts here. We are seeking a skilled Registered Nurse (RN) with OASIS experience for a 13-week full-time contract focused on providing compassionate, high-quality care to patients in their homes.

Schedule:

• Full-time, Monday through Friday
• On-call responsibilities are probable
• Daily visit expectations are clearly structured

Key Responsibilities:

• Deliver in-home nursing care and skilled clinical interventions
• Perform thorough patient assessments and timely, accurate OASIS documentation
• Collaborate with physicians and multidisciplinary care teams to develop and implement tailored care plans
• Execute wound care, manage medications, provide patient and family education, and monitor disease progression
• Maintain consistent communication regarding patient status changes to ensure optimal outcomes

Qualifications:

• Active, unrestricted RN license
• At least 1 year of recent home health RN experience
• Demonstrated proficiency with OASIS assessments (required)
• Experience with HomeCare HomeBase (strongly preferred)
• Ability to manage a full-time, structured schedule with daily patient visits
• Reliable transportation and a valid drivers license
