Your next nursing opportunity is here: a dedicated, skilled Registered Nurse (RN) is needed for a 13-week full-time home health contract. This position is ideal for an experienced home health RN with OASIS expertise who thrives on providing exceptional care, delivering accurate documentation, and managing a structured schedule.

Schedule:

  • 13-week contract, full-time hours Monday through Friday
  • On-call coverage may be required
  • Daily visit productivity expectations

Key Responsibilities:

  • Deliver in-home nursing care, including skilled interventions and patient education
  • Perform comprehensive patient assessments and precise, timely OASIS documentation
  • Coordinate with physicians and interdisciplinary care teams to personalize care plans
  • Perform wound care, medication management, disease monitoring, and health coaching
  • Communicate effectively with the clinical team regarding patient status and needs

Required Qualifications:

  • Current, active RN license
  • At least 1 year of recent home health RN experience
  • Demonstrated OASIS experience
  • Proficiency with HomeCare HomeBase preferred
  • Ability to follow a full-time structured schedule and manage daily patient visits
  • Reliable transportation and a valid drivers license
