Your next nursing career move starts here. A dedicated Registered Nurse with strong OASIS experience is needed for a 13-week, full-time contract in home health. This is an excellent opportunity for a clinician who excels at accurate documentation, hands-on care, and independence, while collaborating with a professional care team.
Schedule:
- 13-week contract, with potential for extension
- Full-time, Monday through Friday
- On-call coverage may be required
- Daily visit structure
Qualifications:
- Active RN license
- At least 1 year of recent experience as a home health RN
- Demonstrated OASIS documentation proficiency
- Preferred familiarity with HomeCare HomeBase EMR
- Reliable transportation and valid drivers license
Key Responsibilities:
- Deliver skilled, in-home nursing care consistent with physician orders and patient needs
- Complete thorough patient assessments and submit OASIS documentation accurately and promptly
- Collaborate with physicians and interdisciplinary care teams to create tailored care plans
- Provide wound care, medication management, patient teaching, and ongoing disease monitoring
- Update clinical teams regarding patient status or changes in condition
