Key Qualifications:
- Active and unrestricted Registered Nurse (RN) license
- At least two years of experience specializing in hospice or palliative care
- Familiarity with HomeCare HomeBase (HCHB) software is highly preferred
- Strong ability to conduct thorough clinical assessments and communicate effectively
- Deeply compassionate, patient-centered approach to care
- Access to reliable transportation and possession of a valid drivers license
Essential Responsibilities:
- Deliver skilled nursing care to hospice and home health patients
- Collaborate with interdisciplinary teams to develop, update, and implement individualized care plans
- Provide emotional and educational support to patients and their loved ones on disease processes, symptom management, and resources
- Accurately document patient status, care provided, and communications using electronic health record systems
- Participate in on-call rotations as needed and respond promptly to patient needs
- Maintain compliance with all clinical protocols, regulatory requirements, and ethical standards
