We are seeking a skilled Registered Nurse (RN) with proven OASIS experience for a 13-week full-time assignment supporting patients in their homes. This position offers the chance to work alongside an engaged, professional care team focused on exceptional clinical outcomes.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be included
Qualifications:
- Active, unrestricted RN license
- Minimum of 1 year recent RN experience in home health
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Ability to effectively manage a set patient care schedule
Key Responsibilities:
- Deliver skilled nursing care and interventions in patients homes
- Complete comprehensive patient assessments and submit timely, accurate OASIS documentation
- Coordinate with care teams and physicians, updating individualized patient care plans as needed
- Provide wound care, medication administration, disease monitoring, and patient/family education
- Communicate significant changes in condition promptly to the clinical team
- Maintain excellence in documentation and patient outcomes
