Your Next Nursing Opportunity Starts Here. We are seeking an experienced Registered Nurse (RN) with strong OASIS expertise for a 13-week full-time home health assignment. If you excel at clinical care, skilled documentation, and managing a structured caseload, you can make a profound difference in patients lives within their own homes.
Schedule:
- 13-week contract, Monday through Friday
- Full-time hours; on-call rotations may be required
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled interventions
- Conduct thorough patient assessments and complete precise OASIS documentation in a timely manner
- Coordinate with physicians and interdisciplinary care teams to implement customized care plans
- Perform wound care, medication administration, patient education, and chronic disease monitoring
- Update and communicate patient status changes to the clinical team promptly
Qualifications:
- Active RN license in good standing
- Minimum 1 year of recent home health RN experience
- Proven OASIS documentation experience
- Proficiency in HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid drivers license
- Ability to efficiently manage daily patient visits and a full-time schedule
