We are seeking a dedicated Registered Nurse (RN) with strong OASIS experience for a 13-week, full-time contract assignment providing home health services. This is the ideal position for an RN who thrives on autonomy, accurate documentation, and structured daily visits, with the chance to make a genuine impact in the community.

Schedule:

• Full-time, Monday through Friday
• On-call coverage may be required
• Daily visit expectations providing variety and structure

Required Qualifications:

• Current, active RN license
• At least 1 year of recent home health RN experience
• Proven OASIS experience (required)
• Proficiency with HomeCare HomeBase is strongly preferred
• Reliable transportation and valid drivers license
• Able to confidently manage a busy, structured schedule

Key Responsibilities:

• Deliver skilled in-home nursing care to patients based on individualized plans
• Complete patient assessments and enter OASIS documentation accurately and promptly
• Administer wound care, manage medications, educate patients, and monitor disease progress
• Collaborate with physicians and interdisciplinary care teams, updating patient care plans as needed
• Communicate clearly about patient status and changes to the clinical team
• Provide compassionate support and education to patients and families
