Your next nursing opportunity starts with a rewarding home health assignment for a Registered Nurse (RN) with OASIS experience. This 13-week, full-time contract provides the chance to deliver impactful patient-centered care in a flexible and supportive setting.
Schedule:
- Full-time hours, Monday through Friday
- On-call responsibilities possible
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions
- Conduct comprehensive assessments and complete OASIS documentation punctually and accurately
- Collaborate with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Perform wound care, medication management, patient and family education, and disease process monitoring
- Clearly communicate patient status changes with the clinical team
- Ensure all visits and documentation meet quality and regulatory standards
Qualifications:
- Active RN license in good standing
- Minimum of 1 year recent home health RN experience
- Proficient in completing OASIS assessments
- Familiarity with HomeCare HomeBase preferred
- Reliable transportation and valid drivers license
- Demonstrated organization and time management skills for structured daily visit schedule
