Your next nursing opportunity starts here. Were seeking a dedicated Registered Nurse with strong home health expertise to provide impactful care on a 13-week full-time contract. If you excel in clinical skills, thorough documentation, and managing an organized schedule, this role offers the chance to make a meaningful difference for patients.
Schedule:
- 13-week contract, Monday through Friday
- Full-time hours with possible on-call requirements
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care and interventions to patients in their home environment
- Complete patient assessments and OASIS documentation promptly and accurately
- Coordinate collaboratively with physicians and the care team to implement customized care plans
- Perform wound care, medication management, disease monitoring, and patient/caregiver education
- Provide timely updates and communicate any changes in patient conditions to the clinical team
Qualifications:
- Active, unencumbered RN license
- Minimum of 1 year recent experience as a home health RN
- OASIS documentation proficiency is required
- HomeCare HomeBase software experience is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to efficiently manage a full-time, structured daily schedule
