An exciting 13-week full-time contract is available for an experienced Registered Nurse with expertise in home health and strong OASIS documentation skills. This role lets you make a tangible difference, leveraging your clinical abilities while maintaining a structured weekday schedule.

Schedule:

  • Full-time, Monday through Friday
  • On-call responsibility likely
  • Defined daily visit expectations

Key Responsibilities:

  • Deliver in-home nursing care with attention to skilled interventions and patient safety
  • Perform comprehensive patient assessments and complete OASIS documentation accurately and promptly
  • Collaborate effectively with physicians and interdisciplinary care teams to execute individualized care plans
  • Provide wound care, medication administration, health education, and disease management support
  • Communicate patient updates and any significant changes in condition to the clinical team in a timely manner

Qualifications & Experience:

  • Active Registered Nurse license
  • Minimum of one year recent experience in home health RN role
  • Required proficiency in OASIS documentation
  • Strongly prefer experience with HomeCare HomeBase software
  • Reliable transportation and valid drivers license required
  • Comfortable managing a full-time schedule, including daily visits
