Your next nursing opportunity awaits as a Registered Nurse specializing in Home Health. This position is a 13-week full-time contract, ideal for RNs with demonstrated clinical expertise and a strong track record in accurate documentationspecifically with OASIS. You'll play a crucial role in providing compassionate, high-quality care to patients in their homes and be a valued member of a collaborative and professional healthcare team.
Schedule:
- 13-week full-time contract, Monday through Friday
- On-call availability may be required
- Structured daily visit expectations
Primary Responsibilities:
- Deliver comprehensive in-home nursing care, including skilled interventions tailored to patient needs
- Conduct thorough patient assessments and accurately complete OASIS documentation within required timelines
- Collaborate closely with physicians and interdisciplinary care teams to create and execute individualized care plans
- Perform wound care, manage medications, provide disease monitoring, and offer patient/caregiver education
- Clearly communicate any changes in patient condition to the broader clinical team
Qualifications:
- Active Registered Nurse (RN) license
- Minimum of 1 year of recent RN experience in a home health setting
- Previous hands-on experience completing OASIS documentation is required
- Proficiency with HomeCare HomeBase is strongly preferred
- Commitment to managing a full-time, structured schedule with daily visits
- Valid drivers license and reliable transportation
